2024-25 Student Information Form
CMS policies were updated in 2023 to align with the North Carolina Parents’ Bill of Rights passed in August 2023. This form is for collecting information related to those policies. This includes the opportunity to opt in to reproductive health and safety education programs, protected student information surveys, and physical and mental health screenings. The full policy text, the student surveys, and other details are available on the CMS Parents’ Bill of Rights webpage.

You will need your student’s ID number to access this form. You can find your student’s ID number on the ParentSquare app by clicking on the icon with three lines at the top left, clicking on your student’s name, clicking on “Dashboard”, and then clicking on “Overview.”
